Developing an addiction, as the terminology is on the slide, whether it's rapid onset or long-standing gradual onset of an addiction, because some people will literally be addicted to cocaine after the first time they try it, literally, physically and mentally, the whole bit. Some of my gambling clients, in my estimation, go from being recreational gamblers to, we used to call them compulsive gamblers, so I'm pausing and changing my language, to having a gambling disorder the first time they have a big win, and so that's like a euphoric experience times 10, if you can imagine you've been plugging away putting nickels into a machine whenever your aunt and uncle come into town because they wanna go get the cheap buffet and do a little gambling, and let's say you drop a couple nickels in and you win a month's worth of pay in one pull of the handle, the euphoric experience on your brain, as you were referring to earlier about the dopaminergic effect, is as strong and as addicting, to me, as somebody who has a massive hit of cocaine check this website.
So whatever the progression is, whether it's rapid or slow, keep that in mind on the physical sense when you compare it to the conversations that people have about being addicted to chocolate or addicted to Diet Coke, that's a conversation in the culture of my middle aged ladies that I hang out with and I always question and, in fact, they made fun of me because I love to knit so much that they said, "Well, do you think you're addicted to knitting?" And I said, "No, where's the problem?" (laughs) Because one of my mentors said when you're trying to determine if the person's behavior is an addiction or a problem, ask them, "Is it creating a problem in any area of their life?" Like Denise just described conditions of addiction, well, here's a whole lot of the conditions. Well, if it's not a problem in any area of your life, but there's a little bit of a flavor of I'm worried about how much money your spending or somebody is concerned how much time you're spending knitting, okay, as soon as it becomes all-consuming, hoarding, you're losing relationships, then yeah, I could see you could probably develop a knitting addiction. But when people are joking about it and they look to me, because I'm a substance abuse professional, to give them some kind of a diagnostic label, I literally use that term, I say, "If it's not a problem, it's not a problem." But if it is a problem to you or to anyone in any one of those areas of your life, it behooves you to go get it checked out. So, next slide... There is a wonderful resource if you're not already aware of it, the Nevada Council on Problem Gambling, and you can access the Nevada Council by just googling nevadacouncil.org. I gave you the link for the little clip that is being shown, the illustration is a piece of the free video called, "Given the Chance." Just wanna make sure that you all know that the Nevada Council on Problem Gambling has a ton of free resources for counselors and for general lay people who just want more information. This particular clip talks about, it's a video, it's a 45 minute video that you can get for free by simply calling or writing to them and requesting it and it also has a discussion booklet with discussion questions that you can instantly plug in to a group for adolescents or for adults. And this one shows the, I think it's kind of a progressive nature of how youth have been given permission for years to have their little garage or home poker parties and how many times have we heard parents minimizing the effects of gambling? They say stuff like, "Well, we're just glad that Johnny is home with his friends, so we buy him all the snacks that he needs and we get all the poker chips and we set him up because isn't it great that he's staying home and he's not going out and doing stupid stuff like drinking or drugging. That whole brain development piece is scary as heck. - [Voiceover] Thank you and I'm sure all of you, I'm preaching to the choir here, but just a reminder, all of you are aware that a significant number of our younger clients are becoming addicted to prescription medicines. I have been paying more attention in the last probably three, four, five years and asking my clients if they have chronic pain, what they're doing with their prescription meds at home and I'm alarmed at how many physicians, especially in the last five years or so, have been prescribing Vicodin as if it were aspirin for everything from toothaches all the way up the pain foodchain and there's so much of it rolling around that I think it's become quite the party drug and I'm not sure if you wanna talk about that now or later.
- [Voiceover] Well, let's save that for a little bit. - [Voiceover] Okay, good. - [Voiceover] down the road. - [Voiceover] And if that's not a gateway drug, I don't know what is, it's their family's prescription meds. - [Voiceover] Exactly. I wanted to bring this to everybody's attention because I just wanted us to remember about the conditions of addiction, that addiction is not an acute condition like a broken arm that you can set and then it gets better. And I think that we, we as in, people in the United States and even people in our field, will pay lip service to the chronicity of the disease, but then we run into all kinds of barriers to being able to treat people in a length of time that's appropriate for whatever their condition is because we're being stymied by insurance companies, you can't keep them more than three or four weeks or Medicaid, it's no longer quote unquote medical necessity well it may not be, they may not have their jaundice may be under control, but certainly their addiction is not under control. I just like to make sure that we all keep in mind that this really is a chronic relapsing disease and I also like the fact that it talks about process addiction. You may manage to stop using alcohol or other drugs or stop your process addiction for significant periods of time, but for most, the disease doesn't disappear, but rather goes into remission. Should you attempt to resume normal use, quote unquote, you will rapidly return to addictive, out of control use and abuse and very seldom in my whole career, I think I've met, and it's been well over 20 years, one person in all of those years, who actually went back and started using alcohol after they'd had a huge problem and managed to not have a recurring problem. Everybody else that I know that went back to using their drug of choice or being involved in their process addiction has gone downhill rather rapidly, so I just wanted to make sure that we address that a little bit, and also, it's progressive, it gets worse over time. It's not something that gets better. With some drugs the decline is rapid, like methamphetamine, for example, alcohol can be more gradual, but it does get worse and it just causes so many biochemical changes and repeated use causes progressive damage and alcohol is one of the worse drugs you can ingest, it affects every single cell in your body and my clients are always shocked when I talk about how much better heroin is for you, physically, because it's far less damaging to your body as a whole, even though you're likely to go down a really horrible rabbit hole very quickly with heroin, where you might not with alcohol. But heroin is actually not as bad for your body. They have to do lots and lots of things to process alcohol to change it from a poison to something that we can actually drink without dying. So I just wanted to make sure that we touched on the chronicity and the progressive nature of the disease. What happens is that adolescents, their clinical presentation is very different from adults, there's a lot of differences and there are some similarities and with adolescents they are not, generally in danger of losing anything really significant. They don't have jobs, they're not married, they don't have children, they live at home, they have a roof over their heads and food on the table, and transportation of some kind and they don't get how impactful their drug taking and/or drinking is to their lives, all they know is that some adult in their life is saying, "What you're doing is not appropriate and not healthy and you need to get help."
So one of the things that we're very cautious of at Quest is working with kids with motivational interviewing and we use a lot of MI because we want to find out what the child wants to get out treatment. And if it's just to get the P.O. off their back, then it's like, okay, well how do we do that? Well, I need to have negative U.A.. Okay, well to get a negative U.A., what do we need to do? You know, those kinds of things. So it's just really interesting and one of the things that I ask all clients to do, I never ask anybody to quit drinking or drugging for life, that's not kind of my M.O., what I ask them to do sometimes is see if you can stop long enough to make a truly conscious decision about whether or not you wanna continue using. Like, what is your life like when you use and what's your life like when you don't use. I really kind of like that little cartoon. And then the one, the big one next to it with the myriad of drugs and alcohol and syringes, I just thought it was a really good illustration about we can become addicted to such a wide variety of substances, there's just a million things out there that alter our moods and our minds and so I just appreciated the fact that there was cocaine and marijuana and needles and alcohol and a little bit of everything there. so it just gives an example of the wide variety of things that we can, as humans, become addicted to. And then of course the gentleman in the upper left hand corner, looking incredibly distressed, once again, we're back to that phrase about significant distress, what is going on in your life around whatever the behavior is and how has it impacted you and how has it impacted your life and do you want help and what do you want to do about it? - [Voiceover] Thanks, Denise, when I look at that picture, this is Denise Q., some of those things were around me when I was growing up, I would have never thought that that stuff or any of the inhalants were an issue. It wasn't until I got into this line of work that I found out that a person with a mind to go out and have an altered experience is gonna be able to find it in a whole lot of different ways. Do you remember when the little special cap started showing up on the whipping cream because people were huffing the inhalant stuff out of whipping cream cans? - [Voiceover] Exactly. - [Voiceover] Do you still encounter stuff like that in some of your clients at Quest? - [Voiceover] Oh, younger kids. Yeah, our 12, 13, 14 year olds have a more difficult time accessing alcohol and drugs sometimes and the inhalants are so much easier to get, but we've had quite a few kids that are still doing that kind of thing, they're still using gasoline dust-off, any sort of product that they can blow into a paper bag and then practically asphyxiate themselves with. And it's so damaging to their developing brains. That's something that we're gonna address a little bit later on. Early-Game Axe is a great jungler but also a very hard hero to jungle properly with. In order to be very effective, you must stack the camps or double tank. Stacking the camps means that you must pull out of their initial location the neutrals at the exact time the next wave of neutrals have to spawn. If the camp will be empty, another group of neutrals will spawn there, granting you 2 groups. Why do you need this? Because you are Counter Helix. The more hits you get, the bigger the chance to activate. Double tanking is when you pull two groups of neutrals and fight both at the same time. The logic is the same, the more hitters, the greater the chance to activate Counter Helix. Mid-Game By mid-game you should have already farmed most of your core items, preferably all. Now you can really start your tanking career and start helping your teams in ganks, initiating and constantly harassing. The strategy is fairly simple: Start with Battle Hunger, Blink in the group of enemies, use Berserker’s Call and see them melt. When the HP of a hero is low enough, just use Culling Blade for a guaranteed kill. By the time the whole fight is over, your Battle Hunger skill has finished its cooldown, so don’t forget to use it again if needed. This is all you need to do during the mid-part of the game: gank gank gank. Late-Game Late game is about taking out the most dangerous enemies. Because of your high defense and HP, you must kill the supports or carries first that might make a huge hole in your team if left alone. Your ultimate is a great way to quickly dispense 1 enemy, already remaining in numeric advantage in a 5vs5 fight. Your Counter Helix and Battle Hunger won’t do that much damage at this stage, so you must rely on your team and on your farmed items to keep you alive and spinning. Your Blade Mail will ensure some extra damage though, which can be devastating if you have a lot of defense and HP and can take lots of damage. There isn’t much you can do at this stage but keep ganking with your team and pulling that high damage on you. Final Words Axe is an extremely powerful tank and one of the best initiators in the game (provided you have Blink Dagger). His farming and jungling skills ensure that you will have a good gold flow that will provide you with the needed items. HE is not a very hard hero to master, but mastering the farming and jungling mechanics are mandatory to ensure you stay competitive throughout the game. As winter draws to a close, where better than the sophisticated city of Nice to welcome in the first shoots of spring? In the run-up to Mardi Gras, this year’s festivities carry the theme ‘King of the Mediterranean’ and run from February 18th to March 8th. At the centre of the Nice carnival, giant grinning and scowling papier-mache heads are controlled by crews on floats as they move from the Albert Premier hotel along the Place Massena.
